Unlocking the device

With this form you can file a request for unlocking the device.

Before you complete the request below, check whether the SIM card of another operator functions in your device. If the device recognises the SIM card, it is not necessary to complete the form below, because your device is already unlocked.

Type of subscriber *prepaid is a rechargeable number, postpaid is a number that implies a monthly bill

Phone number or contract number
*enter the phone number in the format 6XXXXXXXX or contract number in the format X.XXXXXX to which the request refers

Name and last name *Name and last name of the owner of the user number with which the device to which the request refers was bought

Device name and model

Device IMEI *you can find it by entering the code *#06# on the device - 15 digits without signs

E-mail

Code *If you have already filed a request for unlocking device and the received code is not correct, please enter the code that has been sent or otherwise the box will remain empty.

Service interventions

Comment *If your device had service interventions, please specify the details, otherwise the box shall remain empty

Add file (.xls or .xlsx)

*If you wish to unlock a greater number of devices, add enclosure in excel format, specifying IMEI numbers of all devices that need to be unlocked, otherwise the box will remain empty
